ACCIDENT INVESTIGATION AND REPORTING

Activities that must be done during investigation include acquisition of all

information that would explain the causes of the accidents.

The licensee must prepare an accident report, which shall include findings

of the investigation. The accident report must be submitted to the Authority

within 30 days after the accident.
