ACCIDENT INVESTIGATION AND REPORTING












           Among activities that need to be done before investigation includes:






                 Having a policy requiring all accidents to be reported.






                 Identifying and training the investigator.






                 Assembling the tools for investigation.






                 Deciding on who is/are supposed to read the report and implement the

                      recommendations made.
