➢ Failure of shutter to close thereby emitting

uncontrolled radiation

➢ Stolen or misplace source

➢ Unauthorized operation of radiation devices

❖Unsealed Source

➢ Emergency involve spillage and contamination of

radioactive sources onto workers, working area and

environment.

➢ Usually unsealed sources are in liquid form
