8.0 Investigation and Accident Reports 52
The following items should be included in the emergency accident report to
be submitted to AELB within 30 days after the emergency notification.
❖ Description of the accident.
❖ Actions taken during emergency.
❖ Assessment dose to workers, emergency services workers and relevant
members of the public.
❖ The cause of the emergency, corrective and preventive actions taken.
❖ Any necessary improvement to enhance safety and health.
Note
The lessons from the accident should be communicated to all involved
including necessary improvement taken to enhance safety and health.
