❖ Loss of control of sealed source;

❑ Can occur as a result of failure to keep track the movements or

use of the sources according to procedure, that finally can lead

to accidents or incidents;

• Physical damage;

• Lost, stolen or whereabouts unknown;

• Lead to radiation leak and leakage;

• The source falls into the hand of unauthorised person.
